In line with the NHS Long Term Plan, GP practices across England have begun working together to form Primary Care Networks. A Primary Care Network consists of groups of General Practices working at scale to provide a wider range of services to our patients. These services are all additional to those usually offered by your Practice. Your Practice is able to continue to give you all the personalised support that you have enjoyed and expected. All seven of our Practices are committed to this and feel it is a real opportunity. Primary Care Networks are central to providing integrated services encompassing community, mental health, social care, pharmacy, hospital and voluntary services.

Serving a population of 37,700 patients across the rural network, we will initially be working with Clinical Pharmacists, Social Prescribers and as we move through the next five years Physiotherapists, Physician's Assistants and Paramedics. Introducing these new roles into General Practice will help GPs focus their skills where they are most needed, such as diagnosing and treating patients.
Dr Gareth Rowland, GP, Lapworth Surgery and Dr Mansur Ahmad, GP, Henley-in-Arden have been appointed Clinical Directors of Arden Primary Care Network.
have been successful in recruiting a Clinical Pharmacist and a Social Prescriber, they wil work across all seven Practices. The Primary Care Network team will build rapidly over the next five years.
Sheetal Kotecha, Clinical Pharmacist, will be delivering structured medicine reviews and optimisation of our prescribing systems and safety. Over the coming years she will lead a team of Clinical Pharmacists to support all the Practices across the Network.
Amanda Cogley, Social Prescriber, has been appointed to support isolated patients who need help integrating into the community and to help coordinate our wonderful communities to support those who feel lonely or disengaged. Over the next five years we will build a team of Social Prescribers around her.
During the course of their work, as well as working from your registered Practice, the Clinical Pharmacist and Social Prescriber will work form a central base at Henley-in-Arden Medical Centre; this is the Arden Primary Care Network Hub. We may share your information with these Health Professionals. We will only share this information where it relates to your direct healthcare needs. When we do this, we will always ensure that appropriate agreements are in place to protect your information and keep it safe and secure, the law requires us to do this.
